How to Register for Disability Services

How to Register with the Office of Disability Services (ODS)

Step 1: Meet, call, or email the Director of Disability and Counseling Services, Kim Pearsall. Bring, email, or scan any documentation regarding your disability that will be helpful to expedite the process.

Step 2: Complete an Identification and Disclosure Form to identify accommodations that will help with your academic success. If you are unable to come to campus, you may scan and email the Identification and Disclosure Form, as well as any related documentation to Kim Pearsall.

Step 3: Discuss your experiences with your disability and what accommodations you have used in the past that have been beneficial to you in the academic setting. The Office of Disability Services will let you know if further documentation is required.

Step 4: Once all documentation has been submitted and reviewed, accommodations will be approved promptly, generally within 2-3 business days. Students will receive a welcome letter containing their approved accommodations via their Polk State College e-mail.

Step 5: The Director of Disability and Counseling Services will notify instructors of your accommodations via Polk State e-mail or through interoffice college mail. Your accommodation letters state your name and approved accommodations. Your disability is not disclosed unless we receive written permission from you.

Please note: Students are encouraged to facilitate a conversation with their instructors regarding their accommodations at the beginning of each term.
