Polk State College Occupational Therapy Assistant Program Application Question Form

As part of the application process, the student must answer the stated questions in this form. The form can then be saved and submitted within Etrieve during the application process (February 1 through May 31).

Resume Template

This template can be used to assist with developing a resume. An up-to-date resume must be submitted along with the Occupational Therapy Assistant Application Questions Form and OTA Program: Observation Hours Form during the application process.

Physical Exam Form

This form is to be used after the student has been accepted into the OTA Program. The student must ask the physician performing his or her physical exam to complete the form during the examination; the student should ensure the physician completes all sections before concluding the visit. Once completed, the OTA Program Physical Exam Form is submitted to the Program’s Academic Fieldwork Coordinator before the first day of classes in January. [Note: The OTA Program accepts titer verifications or an immunization record for the information related to infectious diseases.]
