Please follow the steps below to complete your activation.

    • Go to the Polk Portal
    • Click on the View all Cards
    • Scroll to “I NEED A CODE” & enter the school’s name, Student ID (use the Banner ID. It can be found on your Profile in the Portal) and your Polk State College email
    • Retrieve the code from your email and paste it into the personal code box on the previous screen
    • The next question is for identity verification. Enter your date of birth and the last 2 digits of your Banner student ID

***Example: if you were born July 23, 2001, and your student ID ends in 55 Enter: 2001072355

    • Complete the remaining steps/questions & select your refund preference of direct deposit, BankMobile checking account, or Paper check
