The application periods for both the Polk State Diagnostic Medical Sonography and Polk State Radiography programs are open from August 1 through August 31.

Both applications are available online. Students who are interested in the Diagnostic Medical Sonography Program can click here and students who are interested in the Radiography Program can click here.

It is important that students review the prerequisites for the program they are applying to as well as the instructions to access and complete the online application.
