Polk State Lakeland Collegiate High School Application Process

Step 1: Attend an Information Meeting

Join us for an upcoming information meeting to learn more about our school.

All meetings begin at 6:00 p.m.

Step 2: Apply to Polk State College

All applicants must first apply to Polk State College before completing the Lakeland Collegiate application.

  1. Start by submitting the Polk State College Inquiry Form.
    This form must be submitted first. Once the form is complete, Polk State College will email a link to the full college application.
  2. Next, complete the Polk State College application.
    Use the application link sent to your email after submitting the inquiry form.

*If you have taken dual enrollment courses at Polk State College and have an existing college student ID number, skip Step 2 and proceed to Step 3.

Step 3: Take the PERT

All PERT testing sessions will be held at Polk State Lakeland Collegiate High School. PERT scores are required to apply to Lakeland Collegiate High School. Review the testing dates below, then click to register.

Step 4: Apply to Polk State Lakeland Collegiate High School

Once you have completed the Polk State College application and met the testing requirements outlined in Step 3, you may submit the Lakeland Collegiate application during the application window.

High School Application Opens January 1, 2027

The 2027–2028 Lakeland Collegiate application will be available on this page beginning January 1, 2027.

Students are required to provide a current high school transcript reflecting all completed high school credits, including any high school courses completed in middle school. Transcripts must include completed coursework and grades through Fall 2026.

A confirmation message will be sent once the Collegiate application has been successfully submitted.

Admission decisions are contingent upon meeting all academic and testing requirements at the time of enrollment.
