POLK STATE LAKELAND COLLEGIATE HIGH SCHOOL APPLICATION PROCESS

Step 1: Apply to Polk State College


All 9th-grade applicants must first apply to Polk State College before completing the Lakeland Collegiate application.

This form must be submitted first. Once it is complete, Polk State will email you the link to the full college application.

➡️ Next, complete the Polk State College application
Use the application link sent to your email after submitting the inquiry form.


Step 2: Take the PERT Test

PERT scores are required to apply to Lakeland Collegiate High School. Only one PERT testing opportunity will be available on Saturday, April 11, 2026, from 9:00 a.m. to 12:00 p.m. You must sign up below to attend the test.

Step 3: Apply to Polk State Lakeland Collegiate High School

Once you have completed the Polk State College application and taken the PERT test, you may complete your Polk State Lakeland Collegiate High School application.

Students are required to provide a current high school transcript reflecting all completed high school credits, including any high school courses completed in middle school. Transcripts must include completed coursework and grades through Fall 2025.

A confirmation message will be sent once the Collegiate application has been successfully submitted.

Admission decisions are contingent upon meeting all academic and testing requirements at the time of enrollment.
