Step 2: Apply to Polk State College


All applicants must first apply to Polk State College before completing the Lakeland Collegiate application.

If you have previously taken dual enrollment courses at Polk State College and have an existing Polk State College Banner ID, you may skip Step 2 and proceed to Step 3.

This form must be submitted first. Once completed, Polk State will email you the link to the full college application.

➡️ Next, complete the Polk State College application
Use the application link sent to your email after submitting the inquiry form.


Step 3: Take the PERT Test

All PERT testing sessions will be held at Polk State Lakeland Collegiate High School (LAC 1100). PERT scores are required to apply to Lakeland Collegiate High School. Testing dates listed below.

Step 4: Apply to Polk State Lakeland Collegiate High School

Once you have completed the Polk State College application and taken the PERT test, you may complete your Polk State Lakeland Collegiate High School application.

Students are required to provide a current high school transcript reflecting all completed high school credits, including any high school courses completed in middle school. Transcripts must include completed coursework and grades through Fall 2025.

A confirmation message will be sent once the Collegiate application has been successfully submitted.

Admission decisions are contingent upon meeting all academic and testing requirements at the time of enrollment.
