ADMISSION REQUIREMENTS
The following are minimum requirements for consideration of admission into the Physical Therapist Assistant Program.

Acceptance to Polk State College as a credit student with all required admission documents received by the Registrar. |

2. |
At the time of application and admission to the program, a cummulative grade point average (GPA) of 2.00 or above on a 4.00 scale. |

3. |
Complete all PTA course prerequisite courses with a grade of "C" or better in each course. |

4. |
Students must complete any required college prep courses. |
|

The Physical Therapist Assistant Program admits students on a "first-come, first-qualified" procedure and based on seat availability. Students seeking admission to the program must complete an application for admission that can be obtained below or by contacting the Program Director or Academic Coordinator of Clinical Education. Once the application is received, it will be time and date stamped. To be considered for screening by the PTA Program's admissions committee, all the necessary preparatory courses, 50 volunteer/observation hours, and the following courses must be completed with a "C" or better on or before May 31 of the applicant's intended year of entry to the program.

Verification forms documenting at least 50 volunteer/observation/employment hours. The observation hours should be completed in at least 3 different physical therapy settings. |
