 NUR 1010C REQUIREMENTS

 

A minimum score of 60 on the TEAS-V test is required to register for this course.  You must bring the following documents to class on the first day of class.  If you do not have the documents, you will be dropped from the course. Do not bring the original documents; turn in copies. Instructors will not make copies for you.

Read the complete instructions for each requirement thoroughly.  If you have any questions or do not understand the requirements, call the Nursing Department at 863-297-1039 for clarification.

  • COPY OF CPR CARD. This must be from the CPR for Healthcare Providers. You can take the class at a variety of locations in the community; however, be aware that many community courses are for lay people and not healthcare providers.  You can take the class at Polk State College by registering for CEV 2319. Polk State offers multiple sections throughout the year in a one-day class from 9:00 a.m. to 2:00 p.m., normally taught in the Winter Haven campus WMS building. You will need the Basic Life Support Manual, which can be purchased at the college bookstore ($16) or rented from the EMS department ($5). The fee for the course at Polk State College is $40. Do not wait until the last minute or you may not be able to find a course.

  • COPY OF PPD RESULTS. If you have never had the PPD test for TB or if there was a gap of more than a year since your last PPD test, YOU MUST GET A TWO-STEP PPD, which means two separate PPD tests. You first get a PPD and then go back in 48 to 72 hours to get the PPD read. If it is negative, you go for a second PPD at any time from one to three weeks from the first PPD, and you then go back in 48 to 72 hours to get the second one read. If the second PPD is not done within three weeks, YOU HAVE TO START OVER. If the first PPD is positive, you do not get the second one; instead you must get a chest X-ray and turn in a copy of the X-ray report. Or if the first was negative and then you had the second and it was positive, you must get a chest X-ray. If the X-ray is positive, you must submit a report from the physician regarding treatment and the advisability to be enrolled in nursing at this time. You can get the PPD at the Health Department, your physician's office, or most walk-in clinics.

  • COPY OF BACKGROUND CHECK RESULTS AND FINGERPRINTING. For Polk State College to get the results, this report MUST be obtained from our approved vendor and ordered using the information below with the Polk State College Nursing codes.  The background check and fingerprinting for Polk Nursing must be done regardless of background checks or fingerprinting that you may have previously had done. Go to the website www.certifiedbackground.com to order the background check and fingerprinting. Click on Student to place the order. Our code is OL92. Provide credit card information and answer the questions asked. The cost for the background check with fingerprinting is currently $106. Print out the verification of your order, which will have on it a passcode that will allow you to access the background check report. Wait about a week and go back to the same website to get your background results. Print the results and copy to turn in on the first day of class.  NOTE: It can take 7 - 10 days for background check to be completed so you must place the order far enough before the first day of class to have sufficient time for the results to process. 

    When your order has processed, you will see in your Certified account a link of "Things to Do".  Click on that link for the information regarding
    approved fingerprinting sites in the area. Appointments are limited, so don't delay in getting scheduled. You may find an earlier appointment in another location. Once you have completed your fingerprinting, go back to your Certified account to fill-in the day that the fingerprinting was done. The fingerprinting must be completed prior to the start of classes. The results of the fingerprinting will come directly to Dr. Hutcherson, the Director of the Nursing Program. Once the results are reviewed, Dr. Hutcherson will go into your Certified account and mark the fingerprinting as "yes, eligible for clinical". Print this report and copy to turn in on the first day of class.  
  • If you have any charges on your record, you must see Dr. Hutcherson. Her office is WSC 103 (WH Campus, Science Building). Call Denise at 863-297-1039 for an appointment. Please be aware that if you have a felony charge that is drug related or related to charges of fraud under certain Florida Statutes, under current state law you will not be able to get a license in Florida for 15 years following the completion of any sentence or probation related to the charges and thus cannot enroll in the program. If you have felony charges for any reason, regardless of adjudication, the clinical facilities that we use are not likely to accept you for clinical and thus you will not be able to enroll in the nursing program. If you have any questions, see Dr. Hutcherson.

SCHEDULE: If you are taking this course during the Summer term, you will have class two full days a week. Depending on the specific schedule, you may have a clinical day that will be on a different day other than your class day; however, clinical will usually be on the same day as a class day where you will go to clinical instead of class. If you are taking this course during the Fall or Spring term, class may be one or two days a week, depending on the section you register for, and on designated days during the term you will go to clinical instead of class. If you are in an evening section, you will have class two evenings a week, but you will have a daytime clinical, which may be on the same day of class instead of class or possibly another day. Clinical days are normally from 6:30 a.m. to 3:00 p.m. at local nursing homes; however, this may change. Your clinical day and facility will be assigned after the start of class. If you are a paramedic on the EMS 24/48 schedule, you need to contact Dr. Hutcherson at ahutcherson@polk.edu regarding your schedule.

 
You will need a pair of scrubs for clinical. The approved scrubs will be red top (Cherokee style 4700 or 4777) and black pants (Cherokee style 4001, 4100, or 4112). For warmth, you may wear a plain black or white long sleeve tee-shirt under the scrubs. You will need a good pair of plain, black or white leather shoes with lace-up or Velcro closure and no heels, open toe, or open heel. The sole of the shoe should not be heavily ridged such that it looks like a sports shoe. You will need a plain watch with a second hand. For class, you will wear scrubs (scrubs of choice); tennis shoes are OK. It is permissible to wear the clinical scrubs to class. You do not need a Polk State College patch on the scrubs. You will need a Polk State College student ID from Student Services.
