Polk Community College
maintains a permanent record of academic information and of supporting
and relevant information for each student. The Registrar is designated
as the custodian of student records and is responsible for maintaining
accurate and readily available student academic records. The
Registrar is also responsible for the use and release of information
contained in those records. The College is committed to the protection
of student rights and privacy of information. It complies with
the provisions of the Federal Family Educational Rights and Privacy
Act (FERPA), State of Florida law and State Department of Education
rules. Polk Community College is a Directory College under FERPA
and may release directory information to requesting parties.
The next section outlines students' rights under FERPA.
Students may order official transcripts by submitting
a written request to include: the name while at PCC, social security
number, date attended, address of recipient of transcript, and
student signature.

and new driver license or other picture I.D
NOTIFICATION OF STUDENT RIGHTS UNDER FERPA
The Family Educational Rights and Privacy Act (FERPA) affords
students certain rights with respect to their education records.
They are:
(1) The right to inspect and review the student's education records
within 30 days of the day the College receives a request for
access. Students should submit to the Registrar written requests
that identify the record(s) they wish to inspect. The Registrar
will make arrangements for access and notify the student of the
time and place where the records may be inspected. If the records
are not maintained by the Registrar, the student will be advised
of the correct official to whom the request should be addressed.
(2) The right to request the amendment of the student's education
records that the student believes are inaccurate or misleading.
|
|
Any grade dispute
or request to change a grade to a withdrawal or drop must be
submitted by the student within the term semester following the
semester the grade was earned.
Students may ask the College to amend a record that
they believe is inaccurate or misleading. They should write the
Registrar, clearly identify the part of the record they want
changed, and specify why it is inaccurate or misleading. If the
College decides not to amend the record as requested by the student,
the College will notify the student of the decision and advise
the student of his or her right to a hearing regarding the request
for amendment. Additional information regarding the hearing procedures
will be provided to the student when notified of the right to
a hearing.
(3) The right to consent to disclosures of personally identifiable
information contained in the student's education records, except
the extent that FERPA authorizes disclosure without consent.
One exception
which permits disclosure without consent is disclosure to school
officials with legitimate educational interests. A school official
is a person employed by the College in an administrative, supervisory,
academic or research, or support staff position (including law
enforcement unit personnel and health staff); a person or company
with whom the College has contracted (such as an Attorney, Auditor,
or Collection Agent); a person serving on the Board of Trustees;
or a student serving on an official committee, such as a disciplinary
or grievance committee, or assisting another school official
in performing his or her tasks.
A school official
has a legitimate educational interest if the official needs to
review an education record in order to fulfill his or her professional
responsibility.
Upon request, the College discloses education records
without consent to officials of another school in which a student
seeks or intends to enroll.
(4) The Family Educational Rights and Privacy Act (FERPA) authorizes
the campus to release "Directory Information" concerning
students. The campus may release this information at any time
to any requesting party unless the campus has received notification
from the student.
The following categories
of student information are designated as "directory information":

- Previous institutions attended
- Major field of study
- Degrees conferred (including
dates)
- Honors and awards
- Participation in officially
recognized activities and sports

(e.g. height, weight)
- Photographs
- Students may refuse to permit
disclosure of this information. To do so, a student must complete,
sign and submit to the Office of the College Registrar a request
to withhold directory information. The request may be made at
anytime and will remain in effect unless and until the student
submits to the Office of the College Registrar a request for
it to be removed.
If the student does not complete and submit the
appropriate form requesting the withholding of directory information,
the College will assume he or she consents to the disclosure
of such information.

with the U.S. Department of Education concerning alleged failures
by Polk Community College to comply with the requirements of
FERPA. The name and address of the Office that administers FERPA
