Polk State College honors the State of Florida's Office of Supplier Diversity's (OSD) certification program. We ask that you inform the College if you believe that you may be able to meet the criteria of the OSD, so that we can ask them to contact you directly regarding receiving your certification. It is very easy to complete our form. Just follow these instructions:

Print the Minority and Woman Owned Business Declaration form, and check either:

Black American
Hispanic American
Native American
Asian-Pacific American
Asian-Indian American, or
Woman Owned Business Enterprise


Include the name of your firm, address, phone, fax number, and EMail address. Mail the completed form to:
