Form 1098-T provides details about your HOPE Scholarship & Lifetime Learning Credits. If you did not receive or have misplaced your 1098-T you can quickly access the information online. If you don’t have a computer with Internet access, PCC can provide a replacement 1098-T.

Here’s how…

To Access 1098-T Online: Go to PASSPORT and log-in by entering your Social Security number (without spaces or dashes) and a PIN number. (If you need help using PASSPORT see our PDF instruction booklet.)

Once you’re logged into PASSPORT, click on 1098T in the Menu Bar at the top of the screen. Then select Display 1098-T Form and the HOPE Scholarship & Lifetime Learning Credits information will appear.

To Request a Replacement: Follow this link to download and print the form: Replacement Request Form (1098-T). You may need a copy of the Adobe PDF Reader in order to view and print the form. (The Adobe PDF Reader is a free program and can be downloaded here.)

Replacement Request Forms are also available at the Cashier's Office on the Lakeland and Winter Haven Campuses.
