Admission Requirements
Minimum Requirements for application to the OTA Program

1. Acceptance to Polk Community College as a degree-seeking student, with all required admission documents received by the Registrar.
2. Completion of any College Prep courses.
3. An overall grade point average (GPA) of 2.0 or higher at the time of application and admission.
4. Prerequisite course work in progress during
the fall semester or completed in previous
semesters:
BSC 2085C Human Anat. & Phys I 5 cr. hrs.
PSY 2012 General Psychology 3 cr.hrs.
ENC 1101 College Composition 3 cr.hrs.
    11 cr.hrs
5. A GPA of 2.00 or higher in all completed pre-requisite courses or courses in progress with a grade of "C" or better in each course.
6. Completion of 20 hours of observation in an occupational therapy department, with direct supervision of an Occupational Therapist or an Occupational Therapy Assistant. Observation hours need to be documented on the "OTA Program Observation Hours Form" (available at the OTA Program). A list of clinical sites available for observation hours may be obtained from the OTA program by calling (863) 297-1010, Ext. 5753. clicking on the following links:

7. Medical Exam documenting the absence of contagious diseases.
8. Submission of a complete and accurate "Occupational Therapy Assistant Program Application" to the Program Manager. The program has a rolling admission policy: students meeting minimum admission criteria are accepted on a first-come-first-serve basis and those students who do not meet the deadline for a particular class will be automatically placed on the list for the following scheduled class.
